NYC Disciples In-Person Worship Registration Form
Hello, everyone!!!

We are SO EXCITED to worship together and see you IN-PERSON. Please read everything in this form carefully, as we have taken great strides to make sure everyone is completely safe during our in-person worship services!!

Registration is one-time only. Follow all instructions below and submit any required documentation. After registering, you will be welcome to join all future worship services. However, COVID guidelines will need to be followed in accordance with CDC guidelines.

Registration Instructions (Please Read Carefully)
Registration for in-person service is required for every adult 18 years of age or older AND any unaccompanied minors (children under the age of 18 who will not have a parent or guardian present).

This requirement includes married individuals who are attending the service with their spouse. Therefore, please understand, *each spouse MUST complete their own, separate & individual registration form in order to attend.*

If you are married with children, only one spouse needs to include the number of children that will be attending. The other spouse can mark 0 children. This will ensure that children are not double-registered.

Upon arrival, ushers may be at the door to check your registration. If you are an adult (over the age of 18) or an unaccompanied minor (under the age of 18), you could be denied entry to the worship service if the ushers cannot find your name on the registration list.
